Thank you for considering Madonna Heritage Assisted Living Home. Our staff is here to help every step of the way and will do everything possible to make your loved one’s move to our exceptional senior living community as easy and comfortable as possible.
We invite you to read about the key steps in our admission process below and contact us at 410-557-9640 or reach out to us via our web form.
A Visit to Madonna Heritage
We strongly recommend that everyone involved in your move visit our community prior to your move-in. Visits during regular business hours are best as you will be able to meet our staff. If your schedule permits, we would be happy to have you join us for lunch. Just give us a day’s notice so that our dining room staff can make necessary arrangements.
The form in this packet along with your deposit and a one-time assessment fee constitutes a complete Resident Application. State regulations require that this form be filled out completely.
Health & Functional Assessment
The purpose of the state required assessment is to ensure that Madonna Heritage can meet the medical and social needs of each of our residents. To arrange an appointment, just give our admissions team a call at 410-557-9640. We will be happy to make the necessary arrangements.
All residents must have a physical exam within 30 days prior to admission. The exam must be performed by a physician and must include a tuberculosis (TB) test or a chest X-ray and completion of the State Physician Assessment Form. The completed form must be submitted to Madonna Heritage within 48 hours prior to admission.
Madonna Heritage will need to have copies of the following documentation on file. Please provide these documents and our staff will make the necessary copies and return the originals to you.
- Power of Attorney
- Living Will
- Do Not Resuscitate (DNR) Order
- Medicare Cards
- Private Insurance Cards
- Funeral arrangements
- Guardianship Papers
An Admissions Agreement outlining services offered at Madonna Heritage completes the admission process. The term of the agreement is monthly but can be terminated under certain conditions with 5 days written notice to the Director of Admissions.
